There are some required documents the school will need to be able to register your student. Please check the list below...

1. Original birth certificate
2. A copy of the students most recent physical and immunizations
3. If student is not living with parent, proof of legal guardianship must be provided
4. DCF 603 form if a foster placement

 Proof of Thompson Residency (provide one):

1. Mortgage statement or rental/lease agreement
2. Utility bill (such as gas, electric, phone)
3. A Certificate of Residency form needs to be filled out and must be notarized if the student and the parent/legal guardian will be residing with another Thompson resident.
